What is NDIS Support Coordination?

You can think of Support Coordinators as helpers who can assist with choosing service providers, connecting you with community services, and making sure that a NDIS plan is working for an individual's needs. This could be anything from finding the right specialist like a support worker, physiotherapist or a dietitian, or connecting you with a social group or activity in the community.

There are three levels of support, ranging from:

  1. Basic support connection, helping you to connect with local services and support

  2. Support Coordination, where they are more involved in helping you access and manage all the services and support

  3. Specialist Support Coordination, for people with more complex needs and who needs a higher level of support

How and what your support coordinator can help with

Your support coordinator can help with some or all of the following tasks:

  • Help understanding how the NDIS works, including your plan so you get the most out of it

  • Plan and coordinate your supports, including finding local services that meets your needs, and arranging when they visit you

  • Establish and maintain your support services, including how much you pay for them

  • Help find new support services or providers, or change providers if they no longer suit you

  • Make changes to your new plan and help you plan reassessment

  • Reporting to the NDIA, including plan implementation and mid-term implementation reports
