Alliance Community are a NDIS registered service provider specialising in delivering quality, individualised and flexible support services to the elderly, mental health clients, young and old people living with a disability. We also provide a broad range of services to clients outside the NDIS including Veterans Home Care program and Aged Care.

We’re looking for an experienced Registered Nurse to work with clients of all ages with complex needs and challenging behaviours mainly within our NDIS case load in in Unley, SA.

About the role:

You will play a crucial role in delivering comprehensive care to clients with a broad range of health needs. The role will work within a multidisciplinary environment where collaboration and patient centred care are at the forefront of everything we do.

The position is flexible either part time 30 hours or full time 38 hours, however working a Friday is a must. We are located on Greenhill Road at Unley. You will be required to travel to see clients in most areas of Adelaide with a kilometre reimbursement.

We are looking for:

  • Holistic care and wellbeing of customers in their home
  • Liaise with internal and external stakeholders including family members, Support Workers and allied health professionals to support clients
  • Clinical assessments for new clients
  • Clinical reviews every 12 months at a minimum, or sooner if health needs have deteriorated or other reasons deem appropriate
  • Liaising hospital discharges
  • Clinical incident investigation
  • Creation of Individual Care Plans for clients

Qualifications:

  • AHPRA registration
  • Batchelor of Nursing / Transcript
  • Minimum 1 year experience in a community setting
  • Current First Aid / CPR
  • National Police clearance, NDISWC and Working With Children Check (or willing to obtain)
  • 3x Covid Vaccinations
  • Excellent interpersonal, communication and problem-solving skills
  • Ability to work autonomously
